OSHC Co-Curricular Permission Form

My child is registered to participate in co-curricular activities for 2024.

I give my child permission to attend co-curricular activities while attending OSHC.

I acknowledge and understand that during any co-curricular activity that my child will be under the direct supervision of their coach/teacher and under indirect supervision of St Pius X OSHC. Once the co-curricular has finished direct supervision will return to the OSHC service.

I acknowledge that my child must sign in with OSHC before and after they attend their co-curricular.
