ONLY Complete this form if your child has a diagnosed Medical Condition requiring a Medical Action plan and/or Medication for the 2025 school year.
Plans and medication must be provided to Student Services Officer (front office) or OSHC Director prior the first day of attendance for 2025.
If there is any change in your child's medical needs/condition throughout the year please complete this form again.
Student Action Plans
Student Action Plans must contain a review date and be reviewed by this date by a medical practitioner. If your child's plan expires or is not reviewed by the review date noted on the plan, your child will be unable to attend school or OSHC.
Medication
Medication to accompany the Action Plan, must be provided by parents, be the exact medication listed on the plan and be in date. It must contain a pharmacy label with your child's full name and dosage required.
Communication Plan
Relevant staff and volunteers will be informed of your child's Medical Condition / Health Care Need and Risk Minimisation Plan.

Whilst in OSHC your child’s Medical / Health Care Plan will be displayed in an area accessible by staff who are responsible for your child’s education and care. To comply with the Education and Care Services National Regulations your consent is required. The information will not be used for any purpose other than to ensure the wellbeing of your child.
